چکیده
Extracorporeal life support (ECLS) devices are lifesaving for critically ill patients with multi-organ dysfunction. Despite this, supported ECLS at high risk ECLS-related complications, including nosocomial infections, and mortality rates in this patient population. The suspected to be, part, a result of significantly altered drug disposition by the circuit, resulting suboptimal antimicrobial dosing. Cefepime is commonly used serious infections. dosing not routinely guided therapeutic monitoring treatment success dependent upon percentage time interval that concentration remains above minimum inhibitory organism. This ex vivo study measured extraction cefepime continuous renal replacement therapy (CRRT) extracorporeal membrane oxygenation (ECMO) circuits. was studied four closed-loop CRRT circuit configurations single ECMO configuration. Circuits were primed physiologic human blood–plasma mixture dosed achieve concentrations. Serial blood samples collected over concentrations quantified using validated assays. In experiments, rapidly cleared dialysis, hemofiltration, hemodiafiltration, greater than 96% eliminated from 2 hours. circuits, mean recovery similar both standard control. Mean (standard deviation) circuits ( n = 6) 39.2% (8.0) 24 control 3) hours 52.2% (1.5). hemodiafiltration but minimally adsorbed either or Dosing adjustments needed CRRT.

Extracorporeal membrane oxygenation (ECMO) is a life support technique based on modifications of heart-lung bypass technology. It is used to support severe but potentially reversible pulmonary or cardiopulmonary failure. There is increasing use of the technique for neonates and a return of interest in its use for adults. The number of non-neonatal paediatric patients receiving pulmonary support...

A 60-year-old man with history of hypertension and unspecified left ventricular dysfunction had chest pain at home at 9 am. At 1 pm he was transported to a peripheal hospital and treated for acute myocardial infarction. At 4.30 pm, despite pharmacological and intra aortic balloon pump support , the extreme hemodynamic instability and the echocardiographic signs forced the doctors in charge to c...

Extracorporeal life support (ECLS) is a reliable method to support pediatric patients with reversible cardiorespiratory failure associated with congenital heart disease, respiratory insufficiency, or after cardiac surgery. In 2010, our institution adopted an infant/pediatric extracorporeal membrane oxygenation (ECMO) circuit that contains a magnetically levitated centrifugal pump, polymethylpen...

During the course of extracorporeal membrane oxygenation, patients are at constant risk of exposure to air emboli. Air emboli may enter the circuit during routine lab sampling, medication administration, air entrainment through the venous cannula, or via a circuit disruption. Circuit components have been designed and positioned to minimize the quantity of air that travels through the arterial l...
